Transcript Request

Please fill out the following form when requesting a transcript. Please note that all transcript requests will be processed within three business days. Unofficial transcripts can be sent directly to an enrolled family free of charge at any time. Official transcripts, which are signed and sealed, can be sent directly to a school or to the family, as requested.

For students who are not currently enrolled with Oak Meadow, there is a $5 fee for each transcript request. Payment can be arranged by using paypal on the confirmation page.

Please note: A paypal account is not required. Select "Pay with a debit or credit card" on the payment screen after selecting the number of transcripts you wish to purchase and clicking the Buy Now button..
